About Us

ClearConsent is part of the Mercy Gate Federation, a physician-led nonprofit management system. Our mission is to make informed consent truly informed — protecting patients while empowering clinicians.

Making informed consent truly informed

ClearConsent exists to protect patients while empowering clinicians with tools that fit real-world workflows and regulatory requirements.

Protecting patients. Empowering clinicians.

ClearConsent exists to make informed consent truly informed. Traditional consent processes are often fragmented, hard to audit, and do not always reflect whether patients actually understood what was presented.

We believe the solution is a platform that guides patients through genuine understanding, gives clinicians real-time visibility, and gives compliance teams an auditable record of every consent event — not just a signature.

ClearConsent mission

Born inside a physician-led federation

ClearConsent is part of the Mercy Gate Federation — a physician-led nonprofit management and innovation system that links multiple physician-owned corporations under shared ethical, financial, and data infrastructure.

ClearConsent focuses on digital informed consent, while its sibling application Kuluq provides the broader EHR experience. Together, they form a tightly integrated clinical ecosystem designed around patient safety and regulatory accountability.

The platform was created to address a simple reality: traditional consent processes are often fragmented, hard to audit, and do not always reflect whether patients actually understood what was presented.

ClearConsent story
ClearConsent technology and architecture philosophy

One auditable system of record. Not a zoo of agents.

ClearConsent is built around a single canonical principle: a system of record for reasoning and consent, with tightly scoped agentic workflows around it. Every design decision — from how AI outputs are handled to how audit events are written — follows from this.

AI is a tool in a structured workflow — not an autonomous actor.

Every mutation — consent assignment, signature, AI flag — writes to the same audit log.

PHI access is governed at the code level, not just the policy level.

The system is designed to be verifiable by a regulator — not just functional for a user.

Three commitments in every line of code

Patient-centered consent experience

Patient-centered consent

A digital consent experience that is structured, patient-centered, and auditable — built around what patients actually need to make an informed decision.

Clinical ecosystem connectivity via FHIR

Clinical ecosystem connectivity

A platform that connects to the surrounding clinical ecosystem — from EHRs to lab integrations — through standards such as FHIR.

Responsible AI workflows with guardrails

Responsible AI workflows

AI-assisted workflows that support clinicians without crossing into autonomous clinical decision-making — with strict guardrails and full auditability.

ClearConsent clinical ecosystem and integrations

ClearConsent doesn't stand alone — it connects

ClearConsent is one part of a broader clinical data ecosystem. It is designed to interoperate with EHRs, lab networks, and practice management systems through open standards — not proprietary lock-in.

Design choices driven by principle, not just preference

ClearConsent is engineered under strict guardrails around PHI, AI usage, and auditability. Design choices are driven by a canonical core thesis: build a single auditable system of record for reasoning and consent, with tightly scoped agentic workflows around it.

PHI is treated as a first-class sensitive resource at every layer of the system.
AI outputs are advisory only — no autonomous clinical decisions, ever.
Every consent event is logged in a hash-chained audit trail that cannot be silently altered.
Agentic workflows are tightly scoped — the system is a record of truth, not a zoo of agents.

Questions about ClearConsent & our mission

The Mercy Gate Federation is a physician-led nonprofit management and innovation system that links multiple physician-owned corporations under shared ethical, financial, and data infrastructure. ClearConsent is one product within this ecosystem, focused on digital informed consent. Kuluq, a sibling application, provides the broader EHR experience.

ClearConsent is not an e-signature tool. It is a consent-specific platform that structures the entire journey from patient education through comprehension checks, teach-back, and signature — with AI-assisted signals and a real-time clinician dashboard throughout. A generic e-signature tool captures a signature; ClearConsent is designed to ensure the patient actually understood what they were signing.

ClearConsent is a focused, purpose-built product for informed consent. It is designed to integrate with EHRs and clinical ecosystems — including its sibling EHR application Kuluq — rather than trying to replicate their full scope. It operates as a consent system of record that emits structured events to downstream systems.

Kuluq is a broader EHR application that is part of the same Mercy Gate Federation ecosystem as ClearConsent. The two are designed to interoperate: ClearConsent handles the informed consent workflow and emits consent-completion events; Kuluq can consume those events to gate clinical actions on valid consent.

Healthcare data governance requires clear accountability. ClearConsent is designed around one canonical audit trail — not a distributed collection of loosely connected agents or systems. Every consent event, AI output, and clinical action is recorded in one verifiable place. This makes it defensible for medico-legal review and regulatorily auditable in a way that fragmented systems are not.

Design decisions are driven by three principles: PHI is treated as a first-class sensitive resource at every layer; AI is advisory only and never makes autonomous clinical decisions; and all consent events are recorded in a tamper-evident audit trail. These are not aspirational — they are enforced at the architectural level.
